Filing Forms & Fee Schedule
All forms can be submitted online at www.OHBusinessCentral.com. To file on paper, please note that we may only accept single-sided, 8.5 x 11 sheets of paper as the documents are all scanned and imaged into our database after being processed.
Please make checks and money orders payable to “Ohio Secretary of State.”
Send forms to the P.O. Box Number listed on the form. Send overnight express packages to: Secretary of State, Business Services Division, 180 E. Broad St., 16th Floor Columbus, OH 43215.
All filings EXCEPT certifications and UCC filings can be expedited. Expedite filings require an additional fee which is stated within the instruction page fo each of the forms below and may be delivered to the Client Service Center in Suite 103 at 180 E. Broad St., Columbus, OH 43215 or mailed to P.O. Box 1390, Columbus, OH 43216.
