You may print and save these documents for future use. However, it is the applicant's responsibility to use the latest form provided by the City of Hoover.
Please be sure to submit the form in its entirety. Most forms are two pages.
For all forms except the Online Jurisdiction Account Information Form, the original completed copy must be mailed to the City of Hoover.

Tax And Licensing Forms --then click the link for Online Jurisdiction Account Information Form above and you will be routed to the form for online completion and submission.
